I am currently only taking appointments through email, but I check it frequently. 24 hours advance notice is preferred, although occasionally I may be able to see you within 2-4 hours.

Screening is necessary to ensure my safety and well being. Thank you for understanding.

Please include the following info in your request:

1. *** References - provider's name, contact info, and how she might remember you

2. The date, time, and location of your desired appt.

3. Your name and board handle (if you have one) 

*** Unfortunately, I am not currently seeing anyone without provider references. At this time, I will only see people with at least two references. They can be from anywhere within the US and Canada, but they do need to be willing to  answer reference checks.
